KIVORAB DSR is a gastro dual-release capsule containing Rabeprazole Sodium 20 mg in enteric-coated (EC) pellets and Domperidone 30 mg in sustained-release (SR) pellets and is manufactured and marketed by Kivonyx Healthcare. This capsule offers dual therapy for acid-related diseases through two different routes. Rabeprazole, which is a PPI, decreases the secretion of gastric acid. On the other hand, Domperidone is a prokinetic that facilitates stomach evacuation and prevents food and acid from washing backwards into the oesophagus.
This combination is highly prescribed in patients with gastroesophageal reflux disease, peptic ulcer disease, and dyspepsia when there is the occurrence of heartburn, distention, nausea, and a feeling of fullness. KIVORAB DSR comes in a 10×10 ALU-ALU blister pack and is one of the products in the Kivonyx Gastro and PPI franchise range.
Key Features
- Pellet-in-capsule design: the EC pellets protect acid-sensitive Rabeprazole from stomach acid, while the SR pellets release Domperidone gradually
- Smaller capsule size for easier swallowing
- Pellets sourced from Metrochem, as stated by the manufacturer
- 10×10 ALU-ALU blister packing
Uses and Indications
KIVORAB DSR Capsules are prescribed by physicians and gastroenterologists for:
- Gastroesophageal reflux disease (GERD) with heartburn, regurgitation, and acid reflux
- Peptic ulcer disease (gastric and duodenal ulcers) with the presence of nausea or delayed gastric emptying
- Functional dyspepsia with the feeling of fullness and bloating
- Stomach pain, irritation, and burning caused by acid
- Nausea and vomiting in connection with reflux or delayed gastric emptying
- The symptoms of delayed gastric emptying (in diabetic patients as per the direction of the doctor)
How KIVORAB DSR Works
Rabeprazole is a proton pump inhibitor. It is a prodrug that becomes active in the acidic space of the parietal cell, where it binds irreversibly to the H⁺/K⁺-ATPase enzyme (the “proton pump”) that produces stomach acid. Blocking this final step lowers both basal and meal-stimulated acid output, giving the food pipe and stomach lining time to heal. Rabeprazole activates quickly and is cleared largely by non-enzymatic routes, so it depends less on CYP2C19 than some other PPIs.
Domperidone is a peripheral dopamine D2-receptor antagonist. It increases the pressure of the lower oesophageal sphincter, strengthens stomach contractions, and improves coordination between the stomach and the first part of the small intestine, so the stomach empties faster. It also acts on the chemoreceptor trigger zone, which lies outside the blood-brain barrier, to reduce nausea and vomiting. The SR release spreads this effect across the day.
Dosage and Administration
KIVORAB DSR is to be taken orally. Normally, it needs to be taken once daily and should be consumed 30 minutes before breakfast when the stomach is empty. The drug should be taken without crushing, chewing or breaking the capsule, since it contains an enteric coating. The dosage and duration are decided by the prescriber. Domperidone needs to be taken at the minimum effective dose for the minimum duration, and any prolongation should be considered by the physician.
Side Effects
KIVORAB DSR is generally well tolerated. Reported side effects include:
- Headache, dizziness, or drowsiness
- Diarrhoea, constipation, abdominal pain, or flatulence
- Dry mouth or nausea
- Skin rash or itching
- Breast tenderness, milk secretion, or menstrual irregularity (raised prolactin with Domperidone)
- Low magnesium, vitamin B12 deficiency, and bone fractures with long-term PPI use
- Rarely, serious heart rhythm disturbances (QT prolongation) with Domperidone, with higher risk in people over 60, at higher doses, or alongside interacting drugs
- Rarely, Clostridioides difficile diarrhoea with PPI use
Patients with persistent or severe side effects should stop the medicine and consult their physician.
Precautions and Warnings
- Under the strict guidance of a doctor only; symptoms like weight loss, trouble swallowing, throwing up blood, and black stools require a proper check-up since PPIs may cover up the symptoms of gastric cancer.
- Make your doctor aware of any heart diseases, heart rhythm disorders, and electrolyte imbalance before taking rabeprazole.
- Should not be taken along with medications that lengthen the QT interval and are inhibitors of CYP3A4 (like ketoconazole, erythromycin, clarithromycin).
- Prolonged use may require regular checking of magnesium and vitamin B12 levels.
- Rabeprazole may decrease the effectiveness of those drugs that require gastric acid (ketoconazole, itraconazole, iron) and may interact with warfarin and methotrexate; discuss other medicines that you might be taking.
- Use rabeprazole cautiously in hepatic disorders and elderly people.
- Use only if it is prescribed by the doctor, especially during pregnancy and the breastfeeding period.
- Should not be used by children without any medical recommendation.
Contraindications
KIVORAB DSR is contraindicated in patients with:
- Known hypersensitivity to Rabeprazole, other PPIs or benzimidazoles, or Domperidone
- A prolactin-releasing pituitary tumour (prolactinoma)
- Gastrointestinal bleeding, mechanical obstruction, or perforation, where stimulating stomach movement may be harmful
- Moderate to severe hepatic impairment
- Known QT prolongation, significant electrolyte disturbance, or underlying cardiac disease such as heart failure
- Concurrent use of QT-prolonging drugs or potent CYP3A4 inhibitors
